Coder Accuracy

 
Coder accuracy is calculated by the following formula:
All errors (except Hard CPT errors) / All chart elements (except Hard CPT errors).
 
In the example below, 1 DX error + 1 resequence error over 3 diagnoses, 1 procedure, 3 POAs, 1 discharge disposition, and 1 possible resequence is
2 / 9 = .22 or 78% accuracy
 
Coder Accuracy
 
All accuracy rows are written using the format of x of y, or errors out of the total of that type.
 
For example, the accuracy display above shows that there are 0 out of 15 DX on the chart.
 
Resequence Errors and Data Transfer Errors are reported differently. We only count a resequence error if the PDX has been resequenced. Data transfers are counted without a denominator.
